Job Description
Job Summary:
We are seeking a motivated, detail-oriented, and compassionate Foreign Medical Graduate (FMG) to join our Chronic Care Management (CCM) and Population Health team . This role is ideal for an FMG with strong analytical abilities and a solid understanding of healthcare systems, both domestic and international. You will contribute to improving patient outcomes by supporting physicians, educating patients, and working with healthcare data and payer information to support quality care delivery.
Key Responsibilities:Assist physicians in delivering primary and chronic care services, including patient intake, history-taking, and follow-up coordination.
Analyze and interpret healthcare statistics to support clinical decision-making and population health strategies.
Work with patient data and payer models (private and government insurance) to assist in identifying gaps in care and ensure payer compliance.
Support chronic disease management by tracking patient metrics, preventive services, and health screenings.
Educate patients on public health topics , self-care, and chronic condition management in a culturally sensitive and bilingual manner (English/Spanish).
Collaborate in the design and execution of quality improvement and population health initiatives .
Document accurately in electronic medical record (EMR) systems and assist in producing care reports.
Understand and support the integration of socialized medicine models and public health frameworks in practice workflows.
Maintain strict patient confidentiality and adherence to HIPAA and all compliance standards.
Medical degree from an accredited foreign medical school .
Bilingual in English and Spanish (required).
Familiarity with U.S. healthcare systems , including insurance payers , CMS , Medicare/Medicaid , and value-based care models.
Understanding of population health , public health policy , and social determinants of health .
Strong organizational and time management skills.
Proficient in Microsoft Office , particularly Excel , and data entry/management tools.
Comfortable using EMR platforms (experience preferred).
Excellent written and verbal communication skills.
ECFMG certification preferred but not required.
Must be authorized to work in the United States.
